How to Nominate a Volunteer:

The deadline for nominations is 17th July 2026

What to include in your nomination:

➢ The name, location and role(s) of the Volunteer you wish to nominate
➢ Why you think they deserve an extra special thank you (minimum of 50 and no more than 250 words)
➢ Your name and contact information

How to nominate:

➢ Complete the Heart of ExtraCare Awards nomination form (speak to a member of staff or download here)
➢ Hand to a member of ExtraCare staff at your location *
➢ Ensure nominations are received by the deadline of 17th July 2026

Further information:

➢Nominations will open on 1st June 2026
➢Each volunteer can receive multiple nominations
➢As a nominator you can only nominate each volunteer once (but can nominate more than one volunteer)
➢Residents, staff, volunteers and visitors can all nominate
➢Individuals cannot nominate themselves or a family member for an award
➢Seven Award winners will be selected by a panel of judges
➢No cash or other alternative is available
➢Only registered Volunteers who are recorded on Portal will be eligible to win
➢Following a nomination, should a nominee no longer be an ExtraCare Volunteer, they may be
deemed ineligible to win
➢Winners will be notified in advance of the award being presented
➢Your nomination entry in the awards signifies your acceptance of these terms and conditions
➢We reserve the right to withhold, change or cancel an award in the event of non-compliance with these terms
